The remainder (67 percent) of ACO leaders or participants have patient-centered medical homes but not clinically integrated networks, according to the 2012 AHA Annual Survey of Hospitals and the February 2014 data release from the AHA Survey of Care Systems and Payment.

Of those hospitals using all three models to transform the delivery of care, 80 percent are affiliated with systems and more than 60 percent are teaching institutions. Hospitals with 400 or more beds account for nearly one-third of this group, and more than 50 percent are concentrated in six states: New York, Wisconsin, Texas, Illinois, Massachusetts and Virginia.
